The slope intercept form is y = mx + b

The equation of line through (x1,y1) and has a slope m is (y - y1) = m(x - x1)

(y - 5) = 6 (x - 3)

y - 5 = 6x - 18

y -5 + 5 = 6x - 18 +5

y = 6x -13

So , the slope intercept form is y = 6x -13
