The equation is y = 5x + 4.

Compare the above equation with slope intercept form y = mx + b.

Slope m = 5.

Say the point  (x1, y1)=(-1, 2)

The line is y - y1 = m(x - x1).

y - (2) = 5(x - (-1))

y - 2 = 5(x + 1)

y - 2 = 5x + 5

y  = 5x + 5 + 2

y  = 5x + 7.

The line is y = 5x + 7.
