find the slope-intercept form for the line satisfying the following conditions

y-intercept 1/5

Line equation with intercepts is x/a+y/b = 1

Here a is x intercept and b is y intercept.

Given a = 2,  b =1/5

Substitute the a,b values in formula.

x/2+y/(1/5) = 1

x/2+5y = 1

5y = 1-x/2

Divide by 5 to each side.

5y/5 = (1-x/2)/5

y = 1/5-x/10

Slope intercept form is y = (-1/10)x+1/5
